Job Description

As an Advanced Early Help Support Practitioner you provide targeted early help support to children, young people and families with specific responsibilities for dealing with more complex cases, including assessment and appropriate management of risks. A key part of the role is developing and sharing specialist knowledge with others in a particular area of practice, and for contributing to practice development.

Responsibilities:

• Manage a complex workload undertaking assessments in accordance with statutory/regulatory and operational standards, policy,and procedures for the service. Plan, implement and review a range of intensive interventions for service users whilst maintaining and providing expertise in early help assessment and intervention, acting as a resource to others within the organisation
• Provide professional, reflective peer supervision as relevant to the needs of the team, and wider service
• Participate and lead in project and development work to promote innovation and excellence
• Maintain accurate, up to date records safely and confidentially in accordance with the Council’s policies and procedures. Contribute to the evaluation and analysis of recording and the use of information systems to inform good practice and maintain a focus on positive outcomes for service users and their families
• Work with individuals, families, carers and communities to help them make informed decisions, enabling them to clarify their needs, construct value for money care packages and commission a range of appropriate services, as well as respond to their feedback regarding service provision
• Undertake preventative work and risk assessments utilising community resources and services such as reablement and telehealthcare
• Provide professional advice, guidance and information to a range of internal and external stakeholders as required and working with multi-disciplinary teams in relation to complex cases
• Liaise, negotiate, influence and collaborate with a variety of internal and external stakeholders, developing effective relationships, resolving conflicts, promoting the service and gathering information as required
• Maintain up-to-date knowledge of best practice, policy and procedure in relation to vulnerable people as well as keeping abreast of wider service developments to enhance the service accordingly to satisfy internal and external demands

Requirements:

• Willingness to undertake the council’s Future Leaders Programme
• Extensive post qualification experience within a relevant field
• Experience of working in an early help service with vulnerable children, young people and families
• Experience of managing complex casework
• Expertise in producing comprehensive and detailed assessments for more complex cases
• Experience of effective multi-agency working
